Exiting Employee Information

The information below should be used for employees exiting your department.

  • COBRA Information: Includes the form and guidelines.
  • Employee Exit Checklist: ES Liaisons and Supervisors are encouraged to use this checklist as a guide in addition to their own best practices when processing terminated, transferred, or retiring employees.
  • Exiting Employee Survey: In an effort to better understand what our employees value most in their workplace and why they leave, we encourage all exiting employees to complete an exit survey.
Please provide exiting employees with private access to a computer and encourage them to provide honest responses to the questions. The confidential information they furnish will be transmitted directly to Employee Services and will be used only to identify patterns or trends in the work environment at the University. The survey should take no more than 10 minutes and should be completed prior to the employee leaving the workplace.
