Early Return to Work: Forms

The forms referenced below are to be used by TAMU departments as part of the Early Return to Work Program (RTW).

Bona Fide Offer of Employment (BOE)
The Bona Fide Offer of Employment form (BOE) is based on a Temporary Work Assignment (TWA) which has been identified by the supervisor/department representative for the employee who is returning to work from an injury or illness. The BOE must describe work that can be performed under the restrictions indicated by the employee’s treating physician. The form must be signed by the employee before the employee returns to work.

Notice of Inability to Identify a TWA
This form is given to the employee if the supervisor/department representative is unable to identify work with tasks in line with the restrictions described by the employee’s treating physician.

Return to Work Physician’s Packet (3 forms)
These forms are given to the employee’s treating physician if documentation is unclear as to whether or not the employee can safely perform the duties of the pre-injury/illness position description.

Physician’s Early RTW Program Information Sheet
This form notifies the employee’s treating physician about the RTW program. This form also provides information about the other forms in the packet and may assist the physician and departmental representative to make informed decisions concerning the ability of the employee to participate in the RTW Program.

Information Release on Ability to Perform Job Duties Form
This form authorizes the physician to release medical information to a specific department representative addressing the employee’s ability to perform the duties of their job description.

TAMU Early RTW ProgramWork Status Report (for non-work related injuries)
The employee will provide this form to the treating physician for those injuries that are not work related.

Important: An employee’s Position Description must be included with the RTW Packet.
